How to print the FIRST forms needed to start a divorce/annulment:

From the website:  http://www.wicourts.gov

Choose “Forms”, then “Circuit Court”, and “Search for a Form

 

CONTESTED WITH MINOR CHILDREN (If you are NOT in agreement); complete these forms:

Summons with Minor Children (FA-4104V)

Petition with Minor Children (FA-4108V)

Confidential Petition Addendum (GF-179)

Divorce/Annulment Worksheet (not found online. Can only pick up at the Clerk of Courts)

Filing fee:  $194.50 (check or cash). Using a card there is an additional card fee.

 

UNCONTESTED WITH MINOR CHILDREN (if you ARE in agreement); complete these forms:

Joint Petition with Minor Children (FA-4110V) – YOU BOTH MUST SIGN THIS AND SIGNATURES MUST BE NOTARIZED.

Confidential Petition Addendum (GF-179)

Divorce/Annulment Worksheet (not found online. Can only pick up at the Clerk of Courts)

Filing fee:  $194.50 (check or cash). Using a card there is an additional fee.

Summons without Minor Children (FA-4105V)

Petition without Minor Children (FA-4109V)

Filing fee:  $184.50 (check or cash). Using a card there is an additional fee. If either party is requesting maintenance/alimony then the filing fee is $194.50
