Children's Long-Term Support Program (CLTS)

When a referral is received, a functional screen is completed by gathering information during a face to face visit with the individual and family as well as reviewing relevant records for the individual. A Service and Support Coordinator (SSC) will be assigned to each individual. Requirements of enrollment in CLTS include regular contact with the SSC, at least two face to face visits per year with at least one occurring in the home, and continued Medicaid eligibility.
